Locating your records when a physician has retired, closed, or relocated their practice.
When a physician retires, closes their practice, passes away, or relocates, their patient records are transferred to a custodian who is responsible for storing them and responding to release requests. The Hamilton Academy of Medicine maintains this directory as the most up-to-date information available for patient charts from its member physicians.
To request a copy of your records, find the physician in the directory below and follow the instructions in the “How to request” column. A standard release form may be obtained from medical offices; if one is unavailable, a written and signed letter of request for copies of medical charts will suffice.
Many custodians listed below receive requests care of the Hamilton Academy of Medicine. The recurring mailing address is: c/o The Hamilton Academy of Medicine, 1 Young Street, Suite 414, Hamilton, ON L8N 1T8. Unless noted otherwise, the standard process is to mail a signed release form together with a cheque for the applicable fee.
